CREW ROSTER COVER SHEET TO BE TURNED IN ON TOP OF ALL FORMS

                     STUDENT or ADULT PARTICIPATION CONSENT FORM
                is REQUIRED for everyone to PARTICIPATE in BurkeChangers!

• Church Registration:
  - Church / Group Participation Fee  $100 Due ASAP
  
   Dates to remember:
  
  
• Registration DEADLINE: May 7th ($50 per participant fee & Required Forms):
  - Student Participation Consent Form     
  - Adult Participation Consent Form w/ Background Check Confirmation
  
  
• Mandatory Crew Chief Training & Job Dispersion:
  - May 2nd 8 a.m to noon (lunch @ CRBA).
  - Rain Date May 30th 8 a.m to noon (lunch @ CRBA).

*ALL Adults MUST  have cleared a Background check w/ Participating Church!

ALL FORMS MUST BE COMPLETED & RETURNED WITH PAYMENT TO:
